Dr. Jain and the entire MediSlim team can help you succeed by offering their medical expertise, support, and motivation when you need it most.
The MediSlim program is not a quick fix to obesity. When you are ready to make a commitment to yourself and to your health to lose weight, then contact us to get started on your weight loss and health journey!

or stop in on Fridays to sign up in person.
We will provide you with program enrollment materials and schedule your initial consultation. The only thing needed to begin the enrollment process is a $50 non-refundable deposit. Any cancellations without 24 hour advance notice will result in a forfeit of this deposit. MediSlim accepts cash, major credit cards, and checks.

You should have all enrollment materials finished and ready to hand in at your initial consultation. The remaining $245 fee will also be due at the consultation. This fee is used to cover your complete physical examination, blood tests, body composition analysis, and EKG ($500+ value!). Dr. Jain/NP Rita Keeling will provide you with a recommendation for your own personally customized weight loss program.

This is where the motivation comes in. Following your consultation appointment it is time to get started meeting your weight loss goals. Patients in the MediSlim weight loss program have regular follow-up visits to check their weight and blood pressure. Any symptoms or issues that emerged during the week can be discussed at this time. Medication adjustments will also be done at this time. The $40 fee for each weekly appointment includes the cost of the most commonly prescribed weight loss medications. Follow up appointments can be made Weekly, Biweekly or Monthly.
Health Insurance Policy:
Many health insurance companies cover all or some of the costs associated with the MediSlim medical weight loss program. If you would like to contact your insurance company then we can assist you with any billing codes. However we do not deal directly with any insurance carriers, and are not connected with any specific insurance plan. Our office can provide you with a letter of medical necessity if it is required by your insurance company.
